All Risk Certificate Program Process

Congratulations you are a current customer of Mover’s Choice with the ability to provide your customers “All Risk” Certificate Coverage. There are three ways you can process your request to provide an “All Risk” Certificate for one of your customers.

All Risk Certificate Form: (PREFERRED METHOD/ON LINE PDF) Your office provides the information directly on the “all risk” certificate and submits it to our office via the internet. Our office approves coverage and returns the complete certificate via email.

Online Request Form: Your office can complete the request form online and submitted it via the internet. Our office will take the data provided, input the information on a certificate, and approve the coverage. The approved certificate will be faxed or email to you within one business day.

Fax Request Form (Download PDF) Your office will download this form, print the form and complete. The completed form can be faxed to our office at (707) 252-5905. Our office will take the data provided, input the information on a certificate, and approve the coverage. The approved certificate will be faxed to you within one business day.
