 The Mover’s Choice independent contractor program is available to household goods and truckload independent contractors who haul exclusively under contract for one motor carrier or national van line. All contractors are expected to be in DOT and CDL compliance.
Individual contractors are not eligible for the plan unless they are signed up through the motor carrier that they exclusively haul for. All cancellations, additions, deletions and billing are conducted through the motor carrier. Motor Carriers will be responsible to contact Paul Hanson Partners directly for these transactions.
The motor carrier will be required to select the occupational injury program they would like offered to their drivers. The following are the options available:
Long Haul Radius Contractor’s: The Mover’s Choice Program provides two options for these types of drivers: Workers’ Compensation Coverage underwritten through Zurich North American Insurance Company or Occupational Accident Coverage underwritten by National Union Fire Insurance Company of Pittsburg, PA (AIG Company).
Local Radius Contractor’s: The Mover’s Choice Program will provide a stand alone state statutory workers’ compensation policy underwritten by Zurich North American Insurance Company. This premium of this policy will be based on estimated payroll for each individual contactor and will be audited annually.
Mover’s Choice has the ability to provide all of the following coverage to the independent contract driver.
Submission Requirements
To submit a risk for review we need a complete application which includes:
- Completed Sponsoring Motor Carrier Application
- Complete List of Contractors which includes:
- Full Contractor Name
- Date of Birth
- Mailing Address
- Social Security Number/FEIN #
- Driver License Number & State
- List of Fleet Driver Information (If Any)
- 1099 Annual Income from Previous Year
- Operating Radius: Local, Intermediate, Interstate
- Legal Entity Type: Sole Proprietor, Partnership, or Corporation
- Past Four Years Loss History by Contractor or Motor Carrier Group –please have segregated by coverage type. (ie. OCAC, Work Comp, Passenger Accident, etc.)
- Vehicle Information by Contractor (If Auto Physical Damage Coverage is Requested)
- Sample of Independent Contractor/Owner Operator Lease Agreement
- Non-trucking Liability and/or Auto Physical Damage Coverage Quotations Available. (Please complete NTL/APD Supplemental Application.)
