Claims Reporting Forms
Use this form to report an auto claim. The transportation loss notice needs to be
completed when an auto accident or when damage to the customer’s physical building
or residential property during move occurs (i.e. damage caused to carpet/hardwood
floor, driveway, stairway, wall, lawn, etc.)
Transportation Loss Notice
This form is designed to be filled out and emailed. The Presentation of Loss and
Damage Form needs to be completed when an Inland Marine Cargo/Warehouse Legal Liability
Loss has occurred. In addition to this completed claim form, we will need: A copy
of both front/back sides of the Bill of Lading and Warehouse Receipt and Inventory
involved in the loss.

Presentation of Loss and Damage Form
To report a claim for loss to Business Personal Property or to Real Property or
to Mover’s Equipment or for loss of business income, complete this form and submit
to our office.
